Required Health Forms for the 24-25 School Year
* Physical and immunization records or proof of appointments must be turned in prior to the start of the school year. Appointments must be scheduled prior to October 15, 2024.
Freshman students must turn in health records/proof of appointment prior to or at the Freshman Rally /Material Pick Up.
Senior students must turn in health records/proof of appointment prior to or at Material Pick Up.
Students that have not turned in proper documentation by August 13th will not be able to start school on August 14th.

All students entering 9th grade are required to have a current physical examination submitted to the MCHS Health Office. A current physical is one that was completed within one year of entering 9th grade. A sports physical does not meet the physical requirement.
New students (new to Illinois), who register after the first day of school have 30 days following registration to comply with the health examination and immunization requirements.
Proof of immunizations against communicable diseases is required by the Illinois Department of Public Health for all students entering grades 9 and 12 and for any student entering District 156 from outside of Illinois.
Below is a list of the documentation required by the State of Illinois for your student to attend school. Printable forms can be found on the district’s website and should be submitted to the Health Office prior to the start of the school year. Students that have not turned in proper documentation by August 15th will not be able to start school on August 16th.
Students entering 9th Grade
Physical Exam (completed less than 12 months ago)
Dental Exam (completed less than 18 months ago)
Polio Vaccine (4th dose after 4th birthday)
Tdap Vaccine (required for all students grades 6-12)
MMR (measles, mumps, rubella) Vaccine (2 doses)
Hepatitis B Vaccine (series of 3 injections)
Varicella (chickenpox) Vaccine (2 injections or proof of having had chickenpox)
Meningococcal vaccine (meningitis) (1st dose on or after 11th birthday)
Students entering 12th Grade
All students entering 12th grade must show proof of receiving an additional dose of Meningococcal vaccine (meningitis) after the age of 16.
All students entering grades 6-12 must show proof of receiving one dose of the Tdap vaccine.

Health Conditions
If your child has a health condition that may require care, please report this information directly to the Health Office. Authorization of all medications brought on school grounds must be verified by the Prescriber, the Parent/Guardian, and the School Nurse. Students entering 9th grade are required to have an oral health exam by a licensed dentist prior to May 15 of the school year.
Submitting Health Forms
All completed health forms can be turned into the Health Office by email, fax, or can be brought into the school during Material Pick-up days in August.
